I think the closest we will get is another feature like Raven Rock in Bloodmoon. You never really are Jarl/Mayor of the city, but you help it along and help define some of the choices it goes through. One of the things I hated in Shivering Isles was the defending the towns around the Isles from creatures. Most of the time, I would get there and wouldn't be able to defend all of the citizens before they were killed off. Same with the vampire attacks introduced in Dawnguard. In Hearthfire, you have to protect your home against bandits, Draugr, rats, and giants. I don't want to be running around from place to place just to defend people and livestock. I want to play the game sometimes just as a traveler, without being tied down by mundane activities. So while I wouldn't mind building a city (like rebuilding Helgen), I don't think I would like to become a Jarl.
Besides, you literally can't become a Jarl without killing off another one. There are only 9 holds in Skyrim, and I can't imagine that is going to change in a DLC.
