It was during the fight with the Arch Curator after he lets out a huge blast I found myself on the edge of a mountain and had to jump my way down, thought it was part of the quest at first and I couldn't use the buttons to bring up a map or weapons, I ended up falling down a waterfall and ended up in the dark passage I think it was called. It was pretty weird luckily loading up a save helped
