Spoiler
- Kill Paarthurnax or let him go free and not be able to continue helping the Blades. This was a very tough one for me because of the nostalgia of the Blades in Oblivion I really, really wanted to help rebuild them. But in the end I just couldn't kill Paarthurnax.
- Become a Nightingale by pledging your eternal soul to Nocturnal. My character might have been a thief, but he was no Daedra worshiper, so this was another tough one.
- Finish the quest and loose the Skeleton Key, or just keep it. Again, another hard decision IMO.

Also, in response to reactions in the world, you do realize that all of those things have to be scripted and voice acted, right? With a world that is this huge and litterally hundreds of different places to explore there are also dozens and dozens of different scenarios that have to be accounted for. At some point, developers (and modders also) have to draw the line and say that's enough content. Sure, would it be cool to have every NPC in the game have dozens of lines of background and dialog commenting on your exploits? Sure, but that is not practical with a game this size. If you let it work for your imagination should be able to fill in some gaps but only of you let it.
