I'm about 28hrs in and with or without the update it'll freeze as soon as I run into a hostile. I've tried sending my companions away and clearing cache and even fast travel all around but as soon as I meet a hostile instant freeze. Please fix this problem or tell me a workaround that doesn't involve starting over. I have also gone back a couple hours to have the same thing happen eventually using auto save or a manual save. Xbox 360 s 250gb with game installed. Please feel free to email me if I can be of any testing needs.

Try uninstalling the game and playing it off the DVD, then clearing cache as well and see what happens. Your HDD may be having issues.
