Havelok kills everyone else because of the guilt he has from doing what he's done. Especially true with the Low Chaos ending.
On High Chaos, Pendelton, Havelok, and Martin are alive, though die through out the last level, as Alvin said, though that stray bullet was fired from Martin trying to get him to come down from the gatehouse. Everyone is basically at each other's throats, fighting for power. Strangely for me, I actually stabbed Martin to death and he never even tried to commit suicide. He did shoot a giant hole in the wall of the gatehouse, not sure what kind of gun he was using, but I wish I had it in the game.
