When I completed the College Of Winterhold questline and then embarked upon a quest to visit the other holds in Skyrim in a quest to learn new spells from the Court Mages.

Probably this. Skyrim have never really made me feel sad, but I definitely felt bad after this part.

Realizing what Savos Aren had done to those poor mages in Labyrinthian.
Kodlak's funeral.
The dead merchants on the road to Solitude, whose bodies are surrounded by Falmer arrows.
Seeing the members of the Dark Brotherhood lying dead in the Sanctuary.

Killing the people in the Ratway warrens, the soilder,and the blind one. By accident, my firestorm was to large I guess.

Another would be killing any really strong enemy, who I fell connected to.
