Has this happened to anyone else.

I was getting chased by Giant Radscorpion and i ran into goodsprings and Victor and Giant Radscorpion started fighting i talked to Victor during the fight and it brought up the companion command menu like he was my companion then the rad scorpion killed him and now everywhere i am supposed to meet victor he is just dead and he is my companion, so i search him and it brings up the companion wheel?

Happened to anyone else?

The potential answer to your question may be a spoiler so if you don't want anything given away, here's a short answer: Your game is not ruined, he will appear again don't worry. He can, I believe, become a temporary companion which may explain your companion wheel bug after he is dead, perhaps the game does not properly regognise he's dead as he comes back.
This has not happened to me, therefore someone else may be able to offer better advice.

There is a glitch for me that I sided with yes man and i killed mr house so victor disapeared i found him though at goodsprings standing their with his face blank i tried to talk to him and it said his artfaficial int. is offline thats not the glitch though after i found him at goodsprings i killed him well with the new patch companions show up on the map well when i look on the map even though hes dead and gone forever he shows up on the map this place north of goodspring cemetary where the radscorpions are but i went and hes not there its just a waypoint i cant get rid of.
