I've got the Azura Star, but not sure how to get souls into it. Do I need to use an enchanted weapon to fill it? i.e. Axe I have enchanted, or the Dwarven Sword of Flames I found?

You need Soul Trap. Either the spell or a weapon. And kill your target within the duration of the effect.

Yes, weapon enchanted with soul trap, unless your a mage. If you want a game given weapon, Mace of Molag Bal does the job nicely.

You need to cast soul trap on an enemy first then kill them and the soul will fill azuras star.

No, they grow in dungeons. You can get a really nice axe in Ironbind Barrow- does fire damage as well as soul trapping.

Azura's Star has to be the only empty soul gem in your inventory too. The Soul Trap spell will fill other empty soul gems before it. Then you recharge something immediately to empty it again or it can't get another soul. I keep Azura's Star and give all the other soul gems to my companion with a soul-trap bow and he helps so we can keep our weapons charged. The traveling Khajiit caravans carry good soul-trap weapons too. I think that's where I ever saw my first one.

The fletcher in Solitude will have one. If not wait 2 days and check again. You can even get the Banish spell that will make you buku gold when enchanted with iron daggers

Actually, a soul that is trapped always goes into the smallest soul gem you have that it will fit in. Azura's Star will hold a grand soul, so if you kill something small, and you have small enough empty soul gems, they will be filled first, not Azura's Star.
