I wouldn't do much. All I'd do is slip in a few diesel powered homebuilt dune buggies, some weapons we would be more familiar with(Just because they haven't been included in a game doesn't necessarily mean they never existed), and I would have quite a bit of semiconductor stuff pop up. I find it quite difficult to wrap my mind how, in 100 years of it's existing, the transistor never quite made it into even mainstream computers. You'd see things like the PipBoy 4000, which was built with semiconductors and is thusly roughly the size of the modern day smartphone(Still wrist mounted, though), you'd see computers with a proper GUI(I would leave in some command line terminals though), color screens, things like that. The PipBoy 4000 would have a built-in cellular phone ability that can be used to communicate with any pip-boy within two miles, and pre-war at any rate, any normal phone in the country.
Everything else would stay the same. Radiation would still ghoulify ya, for example. I wouldn't really change anything crucial to the universe or any of the events in-game.
