Are you talking about an NPC in Fallout: NV or a console command. The only NPC I'm aware of is Pinkerton in Fallout 3. The console command is showracemenu, but will disable all of your perks.

Yasgur speaks truth. I was unable to find any NPC in game that actually does surgery for you which is reinforced by the "Are you sure?" menu when you leave Goodsprings. Console commands exist along the avenue mentioned by Yasgur but I recommend against tampering with the console whenever possible.
