Here are some weird errors I found. Has anyone else experienced these?

Brand new disc and I got a "Disc is Unreadable" error. Had to reboot XBOX
Some sludge pools do not give radiation between Goodsprings Source and Powder Gang Camp
Items in Powder Gang Camp flagged as owned and cause Karma loss when picked up. This should be fair game loot!
Dashed forward after exiting Pipboy, blowing my stealth.
I got the Disintegration Achievement without doing anything. It just appeared that I had disintegrated 32 enemies.

Gripes:
I don't know if he's just really generous or not but nothing in Doc Mitchel's house was labeled as owned, same for one of the goodsprings houses.
My criticals happen all the time. It could be cause I got a really high Perception, but it seems really easy to pop someones head open.
