So being a good person in real life and roleplaying myself each playthrough; I usually walk into Caesar's legion's main base and slaughter everyone including Caesar himself - at round level 12... Now that doesn't really matter, what does matter and annoy me is that it seems to makes zero impact and nobody gives a [censored]. Maybe 1 or 2 generic NPC's on the strip but the NCR and such should be literally bowing at my feet!

Why is that? Sure you killing Caesar probably doomed the Legion to fall apart within a decade or so, but it doesn't solve their immediate problems. Caesar is not the one who is going to lead the attack on the Hoover Dam. What do they care if the legion falls apart in 10 years if they are already dead or sold into slavery by then? If you repel the Legion's attack you get all the bowing and groveling you want. Until then, sure people are happy about it, and many people make mention of it, but they aren't going to kiss your ass because you really didn't do anything to solve their problems yet.

Maybe you don't. Boone will even tell you that Caesar has set up his line of succession significantly. He's planned for his own death, it changes nothing.

The Legate is leading the attack and will most certainly attack with or without Caesar.
