I was thinking - disease is something that should be exploited.
Well the most likely cause of disease, all those gunshot wounds you amass, is probably taken care of, if not explicitly, then implicitly, by the use of stimpaks, doctor's visits or doctor's bags to heal damage. One must assume that those modes of healing would include some form of anti-biotic. And the frequent use of these also means that most other types of bacterial infection would also be negated. That leaves fungal and viral infections as well as other long term diseases like heart disease and cancer. I could certainly see fungal infections being problem, what with nobody ever taking a bath, and unless medicine has come up with viable anti-virals, then we would periodically get sick. I don't know if this is a feature I'd want in a game, since in game terms the mechanic would be: 1. Pop-up says you're sick. 2. Fast travel to doctor. 3. Not sick anymore. Unless some creative game mechanic is used to model something like this, it doesn't seem to add anything to me other than tedium similar to short fetch quest.
