Dual Wielding Glitch [Video included]

So I'm having this weird issue with my character and his ability to dual wield some spells. It's not functionally a huge deal, however it is proving to be very inconvenient, especially in tense battles where I can't afford to think for a second or two, whether or not the right hand is working properly.

The issue that I'm having is fortunately very, very easy to reproduce - which is why I included a video.

Before watching the video, let me describe the issue in detail.
The setup I have is, 1 is fire bolt, 2 is fast heal, 3 is my shield, 4 is my sword, 5 is my bow.
Simple enough.

Now when I just press 1, 1 or 2, 2.. normally both spells will come up in both hands, no problem. You'll see that in the video. Then I press 3 to get my shield and then 4 to get my sword; Again, no problems. However as you see in the video, when I have both my shield and my sword equip and then press for example: 2, 2 to put my fast heal in my left and right hand so I can quickly dual cast it (very important in the heat of battle), the right hand doesn't show up correctly.

Fortunately... the right hand functions as though it's there - at least for healing. When I use it for the fire bolt, it looks almost as though the fireball is coming out from below and behind me, instead of where it should be. Dual charged spells work fine though, they just show the left hand charging the spell and the right hand is still missing, but it's functionally correct.

Now if I have a spell in both hands, right hand missing and I switch the left hand with a spell, the right hand does show up again. The only time the right hand disappears is when I go to dual wielding the same spell in both hands, from a left hand shield, right hand sword.

I have had this problem before as well. For quite some time.

I also have a new problem with summoning spells causing CTDs when cast with my right hand. Specifically the bound sword spell. It works fine in my left hand but causes CTDs when used in my right. You may develop this problem though I am certainly not sure. What mods are you using. Maybe we have something similar that is the culprit?

I know it's a long list, but of all the mods I have - none of them seem to be remotely related to the arms or anything that might be causing this issue. When you said you were having this problem, how did you fix it?

It almost looks like the game is getting caught half way through an animation and not updating the position of the arm when it goes out of screen when it switches from the sword to a hand spell. Any other idea's, anyone?

I normally don't come to forums because I do a lot of research before I deem the problem so unique that no one has an answer. I'm kind of desperate now though, and posting in a forum for an answer is defiantly a last resort for me. I really hope someone can help me out here. :\

This is in vanilla game. I have 0 mods and I have it since 11/11/11.

I don't think we can do anything about it. It's Bethesda's bug.

Is there a way to report it or find out if they know about it?

Or more likely, since bugs usually aren't acknowledged unless a lot of people complain... would someone be able to trace down the issue and maybe make a mod to fix it?
