This will only work under 2 circumstances:
1. You must be using a spell or bow
2. The npc must have a talk option(so no random comments)

You just charge your bow/spell and aim it over them, and then talk to them. The attack will be unleashed, but they won't be mad at you, and guards will ignore it.

Don't know if everyone knows this, I just thought it would be useful

Or just put a bucket on the guards head and crouch while attacking.

We used to be able to do that, but then we took an arrow.... Ow wait no, it was patched
