The closest thing to this that I've had happen to me is when for no reason Cass started speaking with a Protectrons voice.

I had a similar problem in 3. Every time I entered the Citadel, gender not withstanding, only the named members could tell my gender properly.

My character sounds like a chick when he gets damaged as well. Some of the time anyway, it's rather irritating.

Every now and then when my character gets hit he sounds like a female. I just thought maybe he was a wimp, but your post solves my mystery.
Maybe it depends on the age. I had mine on the very least one and I sound like I haven't hit puberty yet.
