I got mine working.

Did everything you said you did. Changed compatibility mode, verified files, changed the .ini file and did the DX9 fix. Nothing. Whats strange to me is everyone else having this problem seems to have never been able to play the game. Mine just suddenly quit working one day. Only thing I can think of that I changed was I created a new administrator account then deleted my old one. Also I don't install the game on my local hard drive, I install it on my other hard drive. But it was installed there when I first started playing. There was a recent windows update that happened and after that I noticed a few things wrong with several programs. But I did a restore and completly uninstalled and reinstalled. I'm gonna try deleting the registry like the other guy said. Well see what happens.

Ok so I uninstalled the game, uninstalled steam, rebooted, Ran REGEDIT and exported a backup. Searched for all files with "Fall Out" "New Vegas" or "Bethesda" (requiring me to uninstall Oblivion as well) . Then deleted all registry that had any of those keywords in them. Im reinstalling now. I did notice something though. When I launch steam it tells me basically that i should run Steam.exe in compatibility mode. I checked the properties of the file and it was defaulted to run in Vista compatability. I disabled compatibility and checked run as admin...and still got the same error. So I just pressed cancel and told Steam to ignore the error. Hope this doesn't have something to do with it. Hopefully it goes well this time, well keep people updated.

Just a wild guess here but check that the other files (falloutprefs and rendererinfo) aren't also checked to be read only?

Have you tried using one of the presets as opposed to using the detected graphics settigns?

Start Steam
Start the NV launcher
Click Options
In Details click Low
Click OK
Click Play

Yes, many times, it does not fix it.

My GF's cousins had a problem with another game not being able to read/write to my documents. Installer was able to create the folder to my document's but the game was unable to write/write. The problem was caused by the path containing non US letters. The path was something like this: C:\Documents and Settings\M?kinen\My Documents\My Games. Creating new profile to windows without non US letters helped. I can't test this because am not having the issue and my profile doesn't contain any non US letters. But I have lots of other issues which makes the game unplayable.

It probably a permissions issue. I noticed the same thing happens to me if i delete the C:\Users\Nathan\Documents\My Games\FalloutNV directory before i click play.
The config files are not being allowed to update themselves. Try to run fallout NV or the launcher (in C:\Program Files (x86)\Steam\steamapps\common\fallout new vegas)
as admin by right clicking and selecting "run as administrator". Also check the security on the folder and make sure you havve the correct permissions on it.

Not trying to be hostile considering you are being helpful, but that is the absolute most obvious fix for this problem that I am sure everyone here tried almost immediately=p

Fair enough. After the video options are set (after you click OK to close out the window), are there two ini files ~20 KB each in C:\Users\Nathan\Documents\My Games\FalloutNV?
