So I was following the coast north/north-east of Solitude (started at the Grotto where you fight the Black Marauders and headed left along the water after). I was just clearing out caves as I have done almost everything else at this point.

I noticed a flag on one of the islands, so I ran over to see what it was. It was a skeleton, hugging a flagpole or the top of a mast, with a flag on top of it. Usually these things are there for a purpose. they show you a chest, or a weapon on the ground, that sort of thing. But..... well, nothing. I had to swim out to the island, fight a half a dozen larger mudcrabs, and then climb a mini-peak to get there, and nothing.

Is he somehow quest related?

Probably just a joke. He's hanging for dear life on a mast, of a sinking boat.

Even FO3 had a guy just laying there with an electric fan against his crotch. Did it have a questline? I hope not!

Yeah, I found it too. There are a couple of islands with flags like that, as far as I can tell they're just fluff.

If it's the same one I'm thinking if it's just a guy with a flag pole who didn't make it to the peak. Should if been a skill book and a weapon with him but that's it.

You'll find a lot of scenes like this one. Most are not related to anything but just for personality and depth to the world.

also reminded me, saw a couple dead imperials in the middle of a road, had some revalers circling them

Maybe it's a joke to those of us trying to discover something that's never been discovered before.
