1. Save the game in front of a terminal during a quest.
2. Use the terminal to complete a task (unlock door, activate turrets, download info e.t.c) then log out of the terminal.
3. Then reload your game save and use the terminal again and you'll just get a blury screen with the noise of the terminal.

The only way to use it again is to quit the game and load it again the everything works fine.
