I made it through the game once on Hard with hardcoe always on. I have to say, it seems like I managed to avoid most of the game crashes that others have encountered, and explored a large portion of the game's optional content.

On my second playthrough, I've encountered a recurring bug. My game crashes when I try to purchase cards for Caravan from a traveling merchant at the Mojave Outpost. I further noticed that cards I have previously purchased to supplement my deck appear in my inventory, but are not available for use when playing the game.

Its a known issue with the cards and the roaming traders. You have to put all of your cards in a container then take them out, that should fix that issue and enable you to purchase cards from caravan vendors without locking up your xbox.
