I have three ideas rattling around in my head.
First and foremost, the opportunity to give the Thalmor a bloody nose and find out more about their inner workings. They're the elephant in the room in Skyrim yet everything seems to work around them. Perhaps the Penitus Oculatus could employ the player on quests to harass, harangue, and sabotage their plots and plans, and we could find more about them in the process and get the chance to one-up them that seemed to be missing from the main quest.
Secondly, the Falmer: yes, there were a few in Dawnguard, but their appearance there was half-baked at best. One of them did say that there were more in caves scattered around Skyrim, however, and we have heard that they're coming to the surface. We could have a questline based around that. An especially bonkers idea is that Calcelmo or an assistant tries to recreate their culture, and wants the player to assist him with a breeding colony; capturing live specimens, weaning them off their fungus food, etc. But it culminates in a break-out and they have to delve deep into Falmer ruins to stop the attacks before Markarth is overrun.
Thirdly, colonisation. We know there are islands to the north and northeast of Skyrim, and that they're inhabited by pirates. Maybe the East Empire Company or some other merchant group (Erikur from Solitude? Perhaps he has holdings on Roscrea, an island already mentioned in in-game books as being governed by Solitude, and wants to expand his empire?) could found an expedition to colonise these islands, a la Raven Rock, and the player signs up? They take part in the construction and defence of a colony in an almost-inhospitable Arctic environment (think a Japhet's Folly-type environment), from wild beasts and Atmoran raiders (Atmora is probably still inhabited: the last ships from there arrived only five hundred years before the events of Skyrim, and even if not Atmorans themselves could have settled on islands further south, or the raiders could be skeletal/giants).
Oh, and spears.
