Realism has to be balanced somewhat with practicality.
For example, followers generally will only die if they get knocked then and hit by you - enemies will ignore grounded comrades (detracting from realism). While its realistic for followers to die in combat, its completely impractical to force the player to reload each time he or she accidently attacks their follower as well as rig it so they deaths
only oncur when the player character accidentally delivers the killing blow.
If a dragon attacks your village, get the hell inside and let the guards take care of it. Damnit alvor, your fists, they do nothing!
