I've noticed that sometimes when I'm playing I'll draw my gun or shoot without pressing the trigger. Very annoying especially when I am near friendly NPCs and I just start shooting them. Controller sensitivity is low, and it has happened even when I purposely keep my finger completely off of the trigger. This has happened with the service rifle and the 10mm submachine gun so far.
