I'm playing on very hard/hardcoe, and this is the first real challenge without using 75 shotgun blast to the head. Is there something I am missing?

Increase you Gun skill? If you aren't enjoying playing it on very hard then just change the difficulty. Plus this shouldn't be in the spoilers section...
