How quickly is reputation suppose to change?

It seems that the reputation shifts in the PC version like to jump too much for each infraction. In Goodsprings, I stumble upon a fire fight between a Goodsprings settler and Powder Gangers. Tried to stay back and watch, they finish the settler and turn on me. I wear them down and finish them in a VATS two for one special. After this, I am informed that I am Vilified by the Powder Gangers. That seems like a ridiculous jump for killing two random wandering gangers nowhere near any other witnesses outside their normal territory. Strange.

Another situation is where I am freeing people from Caesar Legion camp. There's only two guards in the area, so while using a Stealth Boy sneak head-shot one and have to fight the other the hard way. (As for some reason he could easily see me during night hours with a stealth field around me, but that's another issue.) After the first kill, I am informed I am Shunned by the Caesar Legion, then after the second kill... I am Hated. About 30 seconds after the fight and freeing everyone... A Caesar assassination squad shows up to take care of me.

This seems... horrifically broken. No one in QA caught this? Really? *sigh* Am I alone in this issue? If I'm going to be Vilified by any group, I want it to mean something. It should require me to wholesale slaughter swaths of them in front of their base and use their corpses to build a throne over looking their base of operations where I glare cruel, cold into their souls through whatever barriers they construct to keep me out.
