It comes up as soon as you finish the main quest. You will be surrounded by orbs, each of which represents a skill. You can rest your perks in that one skill for a dragon soul.

I believe so. I had no reason to go back and use it again so I cannot say for sure.

BTW it's also tastefully avoidable for those who don't care for the option. It's hard to even notice at first.

Yep. As long as you got a lot of dragon souls you can infinitely respec.
