All you need to know is you cannot play after the Battle of Hoover Dam. The rest of the game is up to you.

Yeah, I don't really know what to tell you. There aren't really any perks that would be specifically good for a beginner. Just play the game and discover stuff on your own. Personally, and I wish I did this before I knew what happened before, I would get a mod to disable quest markers for the main quest. It's so much more fun tracking down the guy who shot you if you don't have a marker telling you directly where to go and you figure it out yourself through all of the information the characters give you.

Exactly, I wish when I needed advice on a quest that the Vault wiki wouldn't spoil it :(

To start out in the doctors office the 2 perks that help the most to get you going are the 4 eye perk and the Good Natured perk.
The Good Natured perk will give you a lot of skills of 5 each and thou losing 5 guns its easy to make it up at your first level up.
The 4 Eye perk will give you 4 perception while wearing glasses and a hat, all which can be found in the doctors office and you can set your perception to 1 and and use those extra points on other attributes you want to have.
Those 2 perks give you a great start skills and extra attributes.
Then have fun go out exploring....

Your build (attributes, skills, traits, perks, et al) will completely depend on your preferred gaming style and projected character concept. Answer these questions and we can help you with more specific information:

1. Do you role-play in depth? IE, do you conceptualize specific character traits and personalities (not nesseccarily what the game hands you vanilla) and then role-play those characterizaions in total? Do you role-play characters like Mad Max, or Eli, or JFK, or a traveling merchant or a Brotherhood of Steel Knight, or anything like that? Or do you just play the character given to you? Do you play that character as yourself?

2. Do you prefer combat centric gameplay? Do you prefer exploration? Do you prefer involved branching questlines? Do you powergame? Do you like to spend a lot of time crafting? Are you a suvivalist?

3. What level of difficulty are you interested in? Do you want your game as hard as possible? Relatively easy? Are you interested in more realistic game mechanics like having to eat, sleep and drink water?

4. You played TES:IV. How many characters did you run through it? One main character? Multiples? How long did it take you to finish the game? How many hours do you average per character?

5. If you're interested in mods, what type of mods interest you the most? Realism mods? Graphics improvement mods? Mods that make combat more challenging? Mods that add extra content like quests, houses, weapons, armors, etc...?

In this game you can play anything from a pacifistic doctor to a murderng sociopath. You can play a cowboy desert ranger, a Brotherhood of Steel Palidin, a Ceasar's Legion Centurian, whatever you want. Answer the above questions and we can help you with some build suggestions.
