I was wondering this this morning, when you level up and choose your special perk, do you keep those for the rest of the game or do you lose it when you select another perk?

You always keep your selected perks.
You could change your traits in the old fallout games with the Mutate perk but other than that you can't really lose a perk once taken.

Yeah. By the time you get to level 30, you will have 15 perks and up to 2 traits.

There are minor perks that you get for completing various achievements.

The challenge perks aren't that strong. It's usually like an extra 1-3% against a particular foe type. Then there are a few misc types of perks that improve other forms of combat.
