Ok so far if you go under the floor in brotherhood of steel place with ED-E he have great chance to stuck on the roof upper you head and glitch him. Other place is Vault 11, there is a underwater path, if your companion come with you, there is a good chance he glitch in the ceilling swimming and bug him.

So what i say, be carefull with ceilling when you have a companion.
