Poor framerate on a new game - please help?

So my girlfriend just got Skyrim on her PC. I don't know her exact hardware but she has windows 7 and it seems to be up to date. She has Direct X 11 blah blah blah... Anyway, I need to know why the game is so choppy even on medium quality and how to fix it. I want her to be able to play it on ultra quality and I'm sure her computer can handle it. I've tried going to the program files and opening the notepad file for "bGamepadEnable=0". A youtube vid said to change it from bGamepadEnable=1 to bGamepadEnable=0 and that would make it faster, but it was already at bGamepadEnable=0. I downloaded an unofficial patch file from skyrimnexus and it didn't really help. also, when I put the settings on 16:10, the only two resolutions are letterboxed... How to I fix the poor framerate and set the resolution right?

**EDIT: I want the 16:10 aspect ratio with 1680x1050 resolution. How can I make this possible?

You need to be more specific than that. What kind of CPU and graphic card does she have? And how much RAM?
