The thing that bugged me was that practically every "dungeon" that was a decent size was quest-related, but being a naturally curious sort I like to explore a building when I find it. This meant that I was often re-visiting places I'd already explored just to grab whatever MacGuffin I'd been tasked with finding next. Too many quests sent me off to explore places I'd already explored and cleared out, often multiple times. I had to do Vault 22 three times, Vault 3 three times, the REPCONN Test facility at least twice...
Off the top of my head the only decent-sized "dungeon" that was entirely optional I can think of was the sewers under Vegas. My advice to anyone starting a new game would be not to bother exploring any large indoor locale unless specifically requested to do so by a quest giver.
