What if a bear could maul the player into a tree. This could occur even if the player had a full set of daedric armor or they just had robes on. This would be an advantage to the creature and would create diversity and difficulty for each play style. Maybe a quick thief would need to dodge the bears charge and a heavy armored warrior could block at the right time and dissipate the blow or they would be thrown off their feet.
Another example: if someone enters a crypt of druagrs. The druagrs could poison the player if they were in their presence. A percent of the players health could steadily decrease making each fight with them challenging no matter how strong the player is. As the player gets higher leveled the stronger druagr could poison the player at a faster rate. Enemies should have a unique advantage over the player which could occur even if the player were very strong.
As it is, the gameplay can be very boring at times. Traveling in the wild, the player encounters a few easily killed, lone wolves--annoyances rather-- a friendly bear, and the occasional giant or two. A sense of fear and involvement is severely lacking, sometimes even non-existent.
