I think that diseases could definitely be cool and make the game that much more in-depth. However makers of the game should use this system lightly. Far Cry 2, for example added a disease factor (only 1 disease - malaria - which could be treated by exchanging "services" for pills). This was hated by some players, and generally liked by others. I think Fallout: New Vegas needs a system for diseases. I think that diseases overall should be a concern reserved for the reckless adventurers (i.e., those who don't take care in some type of hygiene or food preparation). Only in hardcoe mode should diseases be a concern for the everyday traveler, especially for those who are reckless anyway.
Something I think Obsidian should focus on is Radiation poisoning. This was introduced in Fallout 3, and was moderately realistic. A lessened ability to heal during acute radiation poisoning was (strangely) something I enjoyed experiencing. I haven't picked up the good ol' Fallout 3 in a while and I'm not sure whether Bethesda already included this, but a key ingredient in radiation sickness is decreased strength and lessened ability to digest food (less health attained by eating food of any type). I also think that to a VERY LIGHT degree, some types of radiation ingestion should be 'near-permanent,' and decrease VERY slowly over time, decreasing little by little as you get treated by a doctor.
