Really obvious UI mistakes?

I've been playing this game for a day now, and I keep running into really obvious mouse UI issues. e.g. When the game shows you a list of items on the left and details on the right, mousing over items changes the selection... so in order to interact with the panel on the right, you have to carefully move the mouse over without touching any other item. Of course, they put the "accept" and "exit" buttons on the bottom right, so you'll be tempted to move diagonally.

I cannot recall FO3 being this frustrating, so wtf? Was the console UI re-ported over for New Vegas or am I just misremembering?

The game also has an annoying tendency to show you huge lists of inactive items (e.g. recipes), forcing you to scroll down to find the ones you can use. Why not show those at the top?

I also don't understand why the save game system is implemented as it is. The home screen is full of annoying confirmation dialogs even when they don't make sense. On a fresh new install, the game showed me a "continue" option and acted like I was about to wipe out saves even though there were none. When starting a new game, you risk losing an auto-save and have to confirm. Why didn't they simply implement a unique auto-save slot per new game? That would've solved the problem perfectly instead of forcing the player manage their sessions. It seems to be replayability is one of the strengths of Fallout, and yet this is not reflected in any of the design decisions.

That was the same in FO3.
