Wondering if anyone's run into this. After assigning my 16 Skill Points on my transition from Level 2 to Level 3, I pressed the Continue button (square) to choose a new Perk and instead of giving the option of choosing a new Perk, I was immedietely exited back into gameplay without being able to choose a perk. I should have gone back to a previous save and tried it again, but I didn't. Oh well. I'll start over and see what happens.

It's by design. You get perks every second level.
