In the spirit of being realistic...
Consider it a difficulty or a general oversight when developing a FPS game that has a key factor in the gameplay: sound. Is it an honest expectation that you can thoroughly enjoy and appreciate a game such as this 100% through?
Sure, a developer can add subtitles to an opening sequence. However, how would you recommend that option being implemented? Would you see it without subtitles and then realize you need to set that option by closing out, going to the settings, setting the option and then starting a new game? I'm sure someone would complain about that too. If it showed up with subtitles 100% of the time, more people would complain about that.
How about escalation? If such a change was made, what would the next request be in the spirit of accessibility?
Is this really the complaint you wish to make? I would assume you have more important issues to contend with.
For the opening sequence, you're not really missing anything. The visual describes it well enough coupled with the talk with the doctor. Enjoy the rest of the game.
