I've looked through a bunch of the existing threads about this quest's bugs and mine seems to be somewhat different.

Okay...I finished the Companion's questline and became harbinger, but with the Dawnguard expansion I wanted to do the radiant quests to get the Totems of Hircine so I could use the new werewolf howl perks. The problem is that Aela wouldn't give me any quests. So I decided to get a radiant quest from Farkas to see if that would cycle a quest from Aela once I completed the current one.

When I got the "Trouble in Skyrim" radiant quest from Farkas, it took me to Lost Echo Cave which I had already cleared out some time ago. The location had already respawned, but when I enter the cave the quest disappears to the completed section of the journal and wont update. When I come back to Farkas after clearing the cave again, he wont acknowledge that I completed the quest and Aela and Vilkas both tell me that I'm already doing a job for Farkas.

This leaves me in the position of not being able to get the totems of Hircine quests. The oldest save that I have to revert to is six and a half hours and 7 levels of lost progress.

I've been lucky enough with the game to not encounter any game crushing bugs until now, but this one kinda destroys my desire to want to play this character since I had created it to play as a werewolf with the new content. Reverting to the last save would be so much effort to gain back what I lost from such an unexplainable glitch.
