I'm experiencing the "Ice Wraith" glitch seen in the third and last link. I first saw it after killing a group of ice wraiths and lost a good hour of playtime reverting to an unglitched save. I eventually killed another ice wraith and got the same glitch once more. Luckily, I had saved right before entering the area with the wraith and reloaded to find I was somehow still afflicted with the glitch. I tried the save file immediately preceding that (again, prior to encountering any ice wraiths) and still had the glitch.
I have no idea what's causing it; thus, I have no idea how to avoid it. Fighting an ice wraith seems to be enough to bring it about, but I still don't understand how saves prior to encountering the wraith can be subjected to the same glitch. I'm on Xbox 360 so console commands won't help in my particular case. I'd just like to understand what I can do to keep this glitch at bay.

New developments: I noticed something I hadn't before. When the glitch activates, I get a message in the upper left corner of my screen saying "was equipped." I found http://gaming.stackexchange.com/questions/40466/weird-symbol-under-my-character-after-empty-was-equipped by someone who seems to be having the same issue. Like that guy, I got the "was equipped" message right as I used Unrelenting Force; unlike that guy, I was fighting a Snow Bear instead of a Frost Troll.
Nothing shows up under magic effects. I believe this is exclusively a graphical glitch, yet I've no idea what is causing it. Any help would be appreciated.
