I was thinking you could play by rolling a dice everytime your clock reaches 0:00, so you don't decide if you transform. If you do transform you could let yourself have a few minutes to run or hide, or force your character to transform in the middle of Whiterun. You could roll every night, every other night or every full moon.

I believe the ring of hircine as a quest item randomly transforms you, but you can't complete the quest or wear a ring if you choose to do this. I'd rather roll a dice.
