I did role play this almost everyday in Cyrodiil. After all that was the cosmopolitan hub of the world.
In Skyrim I play a real Mountain Man. No washing for me!

It depends on the character. Most of my characters bathe often. They like to bathe soon after a dungeon crawl to get the dust or Chaurus spit off their skin. They also bathe if they've been traveling for a long time. Sometimes they will just go swimming for the fun of it.

Other characters are less concerned with hygeine. They go swimming, but do not make much of an effort to actually get clean. Some characters bathe in lakes and streams. Others bathe only when they are in a town.

When my characters bathe or swim they take off their armor and weapons off and leave them on the bank. This adds a little bit of strategy. In case they are ambushed while swimming or bathing they don't want to be caught without a weapon. So they scout the area before they drop their gear to make sure there are no enemies around.

Not often. Every now and then my characters will head to a pond, remove their clothes and have a bath. But my current character spends a lot of time in Winterhold - most of the water there is frozen, and the stuff that's not is much too cold ;)

My Breton Girl can't resist standing under waterfalls whenever she gets the chance.

I just love going into the water and either going to down the rivers or exploring. I probably should have been an Argonian or at least had some Waterbreathing skills haha.
