Looking at game references it may have something to do with the imperial city, perhaps an extension of the civil war. Maybe the empire tries to break free or the Stormcloaks fight an invasion

Just look at the name! A hearth is basically another name for a fireplace! A hearthfire would therefore mean the fire in a fireplace!

They got a patten on the name dispute it not being important in the base game. They did the same with Dawnguard

Allow to paint a story around them...

The Thalmor-led Dominion is still recovering from the loss at the Imperial City and withdrawing from Hammerfell. Thalmor agents continue to slither through the cracks, searching for any and all means to ensure their victory over the world. Meanwhile, Orsinium has become a bustling haven for numerous Orsimer on the Skyrim/Hammerfell border. Thalmor agents have made contact with the current ruler of the "Iron City", hoping to spark the king's interest in an alliance. Being the silver-tongued devils that they are, the Thalmor attempt to manipulate the Orc king's thoughts by recalling the fate of Nova Orsinium at the hands of Hammerfell/High Rock forces. While consultations are underway, the Thalmor have dispatched expeditionary forces to locate an Elder Scroll. Out of desperation, the Thalmor have uncovered ancient Aldmeri legends deep within the history of their people involving the Orsimer and Malacath. History shows that Malacath was once Trinimac, Auri-El's greatest champion and strongest of the et'Ada. His children, the Orsimer, were once proud and noble warriors of the Aldmeri people. Legend has it that Trinimac skew Lorkhan at the request of his king Auri-El. Through the use of an Elder Scroll, the Thalmor wish to restore Malacath and the Orsimer to their former selves in hopes of once-again uniting the children of Auri-El and Trinimac against the children of Lorkhan - men. Your goal is to journey to Orsinium and convince its king to set aside the wishes of the Thalmor and join the Empire, granting his kingdom provincial status if he chooses to fight against the Dominion. The journey will take you to the Ashpit to learn the truth about Malacath, revealing that it was Auri-El and his children who cast them out as beasts...

Malacath, the once-proud champion of Auri-El, now in service to the Empire, to Akatosh - Auri-El's mirror-brother.

I have a theory regarding Hearthfire.

A hearth is the floor of a fireplace. An ashpit is the receptacle below a hearth that collects ash from the hearth's fire.

Malacath's realm is the Ashpit. Yes, it does seem like a shot in the dark, but allow to explain a few supporting details.

The most important supporting detail is the fact that within the game's files, Malacath has insanely high stats. The files list him at level 100 with over 1200 health, moreso than any other Prince by a huge margin. Why would Bethesda give Malacath such high stats when we can't even meet him? Curious thing.

Orcs have been given a huge overhaul with Skyrim. Bethesda put a lot of time and effort shedding light on their culture. According to the wise woman of Dushnik Yal, Orcs were in Skyrim long before Nords ever set foot in Tamriel. What's even more interesting is that Orsinium has been moved to the Skyrim/Hammerfell border. This is revealed by an in-game load screen, suggesting that it has been rebuilt despite being assumed destroyed according to Lord of Souls. Seems highly convenient.

One of the biggest complaints about Skyrim during its launch was the lack of an arena. Considering what we've learned about the Orcish view of seeking a glorious death, I can think of no better candidate than Orsinium to implement an arena.

I would also like to add that a hearth can also refer to a forge. Forges are obviously associated with blacksmiths and Orcs are known to be the greatest smiths in Tamriel. This is significant because a slew of smithing features could be opened up, such as new weapons like the spear and flail shown in GameJam.

Todd Howard also revealed at E3 once that he would like to see Daggerfall features redone with today's engine. Orsinium was a location featured in Daggerfall...

Considering all of what I've mentioned, I believe that Hearthfire will take us to Orsinium and the Ashpit. I believe it will involve redeeming Malacath. Why? He was once Trinimac, strongest of all et'Ada and Auri-El's champion. It would be in the Thalmor's best interest to recruit Orsinium and attempt to restore Malacath, perhaps via an Elder Scroll. The alternative would be recruiting Orsinium for the Empire, granting Orsinium provincial status.

That's my two cents.

It'd be cool to have the strongholds actually do something and be worth visiting instead of what they are now: A mine with five or six orcs living near it, and a fetch quest.

My guess it will revamp the clearly defunct Magic System. Hopefully it will have nothing to do with ''evil necromancers'', my guess is a high possibility of the Psijiic order appearing. And as others have mentioned Malacath being involved maybe the quest will be about Malacath turning back into Trinimac using the Eye of Magnus. If the Eye of Magnus can unmake the world easily , then maybe it can undo a daedric prince and return it to its original form?.

In the Dwemer ruin of Bamz-Amschend (from Tribunal), one of the areas is called Hearthfire Hall.

No idea if the name means anything meaningful in Dwemer lore, but seeing as how Dawnguard featured Snow Elfs, and Hearthfire has appeared as a name in a Dwemer ruin, perhaps this will be about the Dwemer in one way or another?

EDIT: Not that I particularly want to see them "return", but maybe having a run-in with Fyr and Yagrum or something similar would be interesting.

For me personally i want to see more of what first dlc did. Expand on what is already there. For example for one dlc they could add more quests etc and further develop the civil war with stormcloaks and imperial.

I saw Dawnguard to be like KotN. Not saying it was exactly like KotN, as Dawnguard was far better, but the smaller DLC before we get the big one like Shivering Isles. A large land to explore full of opportunity. At least, I am hoping the next DLC will be Shivering Isles sized.

Some new spell effects across the schools of magic, the ability to build my own home, a Ranger's guild and some new areas to explore.
These things, obviously, aren't going to be put into one DLC, but I'd love to see them spread across a number of DLCs. It likely won't happen, but a man can dream!

Right......for them to sell to us for halloween, the theme is fitting. Not for the nords to celebrate halloween, comprende? A micro dlc expanding upon the existing lore of the falkwreath area, many battles have been fought there, many nords are buried there ( the draugr that would attack falkwreath). Maybe a player owned home could be added on the edge of town as a reward. Typed it real slow ??
