Only one...at my hands. I was more than happy to sacrifice Lydia. Companions/followers are totally useless in Skyrim. The only companions (that I've used) that even come close to being useful and not annoying to the point of killing them personally are Meeko and the stray dog.

After becoming thane of a hold. Killing the HC is the TOP priority. That has to happen. My way of "You're Fired".

Lydia died defending me to the death, just as she promised. I don't want any more followers...can't deal with the guilt...

I killed Lydia myself about 3-4 times. Twice by bow, and twice with the chain-lightning spell.

I always restore a previous save. Eventually I got tired of her dying, so I made her "essential," now I won't have to worry about her anymore. Although she still complains when my spells hit her.

I was doing this Thieves Guild special job, had my beloved housecarl Argis the Bulwark with me. So, I got to this thing... With blades, that goes back and forth, no idea what its called, but I hope you understand what I mean. Soo I go through this, of course, all stupid followers follow you trough it, but then I found nothing that could stop it, so there I watched ol' Argis get cut to pieces (at least he would in real), but that aint the weird thing. Fine, he's dying, I can get a new one, but then one of the blades pushes him, pushes him through the wall, where you obvious cant go. So, there he goes to Sovngarde, with my dragonbone armor, Nightingale bow and so on :) Sometimes, this game really makes sense! :)

Kharjo and Lydia have died too. Oh, and that Marcio guy or whatever he's called in the bee and barb in Riften. There was a thief, so I killed him. The Marcio dude joined in, but then he hit a civilian, making them fight, so there he kills everyone that isnt essential (wich isnt many), and because Im such a hero, I killed him. But the thing that annoys me the most, is that I lost that Dragonbone armor with Argis :(

None of the normal followers, but I lost two dogs, a horse, and the guy from the Kynareth quest, who decided to attack a fort full of bandits for no good reason and deserved what he got.

…poor marcio got on the wrong end of a dragon. :(

Marcio ended up on the wrong end of a sacrifice for me.

I resurrect my followers once, mainly because I'm the one who kills them by accident. After that, they stay dead.

I've had four followers and they've all died except this last dude, the bald guy from Windhelm is still rocking. I'm probably gonna fire him for J'zargo though.

About 3 so far. One woman survived me because I sent her back to her mountain.

I may need her again...

I fed Roggi to a Daedric Prince. Beyond that, they're all still alive; I rarely take them anywhere.

I have had Lydia die on me a few times. I always reload and replay the fight until she survives.

2 Lydia died in a pretty big fight with 2 Draugr Lords and whole bunch've lesser ones. Was a pretty epic battle.

The other was a random dunmer merc I fed to a dark god. Almost a pity I didn't waste time to learn her name...

This is really odd, but Mjoll has not died ever. And that's been in over 30 hours of playing. I did give her a stash of health potions, so maybe she drinks them - does anyone know?!

I did have Erik for a while, but after I met Mjoll, I knew I had to have her - Erik was boring by comparison. And I find Lydia way too obsequious - all that Thane nonsense - get a life Girl! And she's always sitting in my bedroom - why doesn't she go to her room for a while?! Creepy trying to sleep when she's in there.

And wierdly I didn't intend to adventure with others at all - I never did in OB, so I am quite surprised how much I am enjoying it. I also have an immortal dog with me as I haven't finished a particular quest, but I have to say he's constantly getting in the way and nudges in to me, trying to push me off mountains and the like!

I know one way to do it permanently, but you are not up to that point of the main quest yet. You'll realize when you get there, don't despair. Then you may miss her. :)

Lydia was killed by thugs. I'd been bad and went around robbing the houses of Riften, and they over-powered her.
Sven was killed by skeevers. I was in a skeever nest and while my back was turned, they just mauled him to death. He was pretty weak anyway.

I'm being careful on my current follower, Aranea, though. Got her really stocked with healing potions and fancy items so she won't meet the same fate as the other two.

My Lydia died a long time ago, after many successful battles, killed by the m*therf*cking Captain Hargar and his axe of doom. I'll never forget the name of that dude because of all the trouble he gave me.

More recently. Iona died as well, killed by a Falmer using shocks on our way to Blackreach.

Just one.

The brief time my character traveled with Eola ended with her dying. While my Bosmer had nothing against cannibalism after a battle, especially of worthy enemies, he was against the idea of luring a befriended priest he had just helped out to become the main course at a dinner party. So, an arrow got planted in Eola's skull.
