Quest: In My Time of Need
If you side with the girl, and kill the Redguard warriors & leader, you'll later find two redguard warriors lurking just inside the entrance to Whiterun. They appear to be the ones that start the quest chain by getting refused entry to the town. They won't respond to chat, never move or leave, and the area starts to lag badly with them present.
Conversely, if you choose to side with the Redguards, their leader sometimes shows up alone when you prepare the ambush. All his followers freeze up in their lair and become un-responsive. This can cause the quest to be impossible to complete
Quest: Bounty (Dragon Slaying for Jarl of Dawnstar)
After killing the dragon and 'handing in' the quest to the Jarl, you are given the bounty of 500g but there is no 'quest complete' message, and the quest remains in your log permanently. Apparently you can then receive the quest again multiple times, causing it to 'flood' your quest list with no way to complete/remove it.
Quest: Waking Nightmare
If you enter the ruins with a (paid for) companion, he is forced to stay outside. If you then go back outside and tell him to go home, he tells you he can be re-hired for free, and 'you know where to find me'. However, he does not return home - instead he lurks in the area fighting snowcats, and if you re-hire, you'll be charged again - even though he remembers you.
Quest: Forbidden Legend
Inside one of the three dungeons, I think it was Folgunthur, there is a puzzle with four levers and a series of gates. I believe the combination is generated when you enter the dungeon. It is possible for a 'bad' combination to occur, ie it is impossible to have all the gates open at once and thus you cannot proceed or complete the quest. The only way to fix it is to reload a saved game from before entering the dungeon (the auto-save on entry is 'too late') and hope for the best.
edit: To any of the people with really serious issues, ie constant lag, frequent crashes, odd dragon behaviour etc: Have you tried a full re-install of the game and playing with a new character? The design of the game makes it very hard for patches to fix existing problems on a save file, and with hundreds of hours logged I've only had 2 crashes and none of this other stuff. Not questioning anyone's comments, just curious.
