I've never killed everyone in the game, no, I think playing in a world where everyone was dead would get easy pretty quickly. Although I've occassionally just randomly killed people just to watch them die when I'm about to exit the game, I didn't save of course, it was just for fun, I wouldn't risk breaking quests just for a few momentary deaths. I think I've done this twice to every person in the Old Mormon Fort, I wouldn't do this for everyone in the game, though, too much ground to cover for something that I'm not actually going to save, and I'd probably get bored with killing people without reason soon enough. Generally speaking, I only kill people with what I deem a good reason. In RPGs, the general rule is that I only kill non-hostile NPCs if there's a quest involved with them, though I've made a few exceptions if I really don't like them before.
