The most important thing is probably terrain. You need somewhere with protection from breath attacks. I run around like a madman trying to find the best spot. It is also helpful to drag him into other enemies, like bandits or necromancers. Then when he lands, I go berserk, buff attack speed, and run out and swing like a baseball player on crack. However, if he has high health, and you go head on, he is likely to chew you up and spit you out. This is where any kind of companion, other enemy, summon, etc. is good as a distraction.
