The more I play this game, the more I love it. I'm not too far in; limited free time has meant I've only had the odd chance to play it. I did have a nice five hour session on Sunday though.
While the missions, combat and dialogue are all great, what really makes the game for me is the overall atmosphere of the world. There is always something going on, some little event that you can intervene in or simply watch. And they always feel organic; I've never felt like I'm witnessing the same thing twice. Rockstar touched on this in the last few GTA games, but they really hit the nail on the head with RDR. Bethesda should take note for their next TES/Fallout game.
If I could fault the game, it's that I've encountered a few (minor) glitches. Horse and NPC behaviour is occasionally erratic too. But these small problems rarely detract from the game, and compared to some other sandbox games lately (Just Cause 2, I'm looking at you), it's relatively bug free.
I haven't tried multiplayer yet, but I'm looking forward to giving it a go when I have the time. This is a game I can see myself trying to 1000/1000 achievement-wise, and even playing for a while after that.
