I don't think we even have to reach so far back, how about FONV? The karma system and factions were pretty well done imho. I never liked the FO titles post-apocalyptic setting but when I really started playing NV you can really appreciate all the subtlety.

Now imagine if Skyrim had so many different factions and the choice you made about alliances actually mattered.
I wrote a http://j-u-i-c-e.hubpages.com/_esforum/hub/skyrim_simple_fixes that I think they should have made. All of these fixes would have been anywhere from trivial to extremely easy to make. No big fancy new features or overhauls, but if all these points were fixed I think it would cut down on a lot of complaints about the game. Personal opinion, of course.
I would really love the journal to have actually ya know progressed! Its going backwards (again) from having the details we found in MW and to a lesser extent OB. I would like each quest to have the kind of detail that lets me turn off all the quest markers. I love getting directions of varying quality then having to go look for places. In Skyrim the quests are so poorly detailed there are many you could only complete through pure dumb-luck without the f*ing quest markers.
