if FO4 was a real-time settler game, then getting married/raising a family would offer interesting twists and subplots eg one of your family was killed while out scavving by a raider party, or some "Family" type clan, then you could start a feud with that clan, get all the people in your village involved. (I've been very excited about the idea of a Fallout Settler game ever since the Real-Time Settler mod arrived. While that was good, the possibilities for a concept completely written around it would be awesome)
In a quest type scenario like FO3 and FNV, then no, I don't think it would add anything to it particularly, other than giving you someone else to look after. But you get that with followers anyway, and at least they pull their weight (in game terms)
