Getting killed by a mudcrab in morrowind. Drowning because you get stuck in the statics. Also, getting killed by someone using a gauntlet when you're equipped with a bfg in quake 3...

Yesterday I crashed a car into a wall at top speed in Mafia II because someone was talking to me on Steam.

I also tend to fall off high things a lot.

Was playing Chaos Theory coop with a friend, thought we cleared the level out, ran down a hall and a guard slammed a door open as I ran by it. My ragdoll flew into the opposite wall and my friend was too busy laughing to notice he was being shot dead.

Just thought of another..This was in Terraria with the same friend. We were deep in a cave system and came across a small ruin, and I decided it was a fine time to test out my new explosives, so I chucked them towards an opening in the dirt nearby; they ricocheted off a wall and blew me to bits. This was before I reset my spawn, too.

Most embarrasing loss was in Fall from Heaven 2 (Civ 4 mod). I have my starting settler establish my first city, then send my starting warrior to explore a dungeon right next to my city. Doing so spawns several barbarian units, one of them right on my unguarded city. Game over, first turn. I couldn't lose so quickly again if I wanted to.

Happened more times than I'd like to admit

And once in a dungeon, during a bad pull, I tried to use my mind amplification dish to provide some last-ditch crowd control, it backfired, I got mind-controlled and wiped the group. Fortunately, nobody realised what had happened and they all blamed the hunter :P.

It is truly embarrassing how many times I have done this. It happened the most in Gears of War for me.

Dying on the first round on Bronze in Mass Effect 3's multiplayer

Walking over exposed traps in Dungeons of Dredmor

My personal favorite was back when I played WoW and before the Death Knight revamp. My raid had just wiped on Sartharion back at the beginning of Wrath. We all had gotten back except for one of the tanks, whose corpse was at the boss' feet. We all start buffing up while waiting for him to come back, and I cast Raise Dead to bring my ghoul out. Now, the way Raise Dead works is that if there are any nearby corpses, the Ghoul will come out of one of those instead of out of the ground like normal. Guess where my ghoul came out of?

Cue Vent chaos and a mad scramble for the exit portal. My guild didn't let me live that one down for weeks.

Another WoW related one that wasn't my fault. Same guild, same expansion pack, different instance. This time we were near the end of Naxxramas and we decided to take a short break. A few of the people who got back before the others decided to mess around with the RC car items that we had all gotten in the Christmas event that year. One of them drove a little too close to the boss. Hilarity ensued. That guy took even longer to live that one down than I did with my ghoul stunt.

Another one just happened: playing on my Minecraft server with my brother, who got lost in the jungle. So I went to look for him, when a damn Skeleton jumped out and because they are silent and always make me jump when their arrows hit me, and in my haste/surprise I back-peddled into the pit I had only moment ago walked around so carefully and died.

As said above, grenades too. Can't tell how many times in Halo I tried to throw plasma grenades with precision over cover only to have it now quite make it over the edge, bounce off and stick to me.

I've died many embarassing deaths in many different games, but the first one that comes to mind is a classic: falling into a hole/gap in Super Mario games

In CS:S on de_nuke, trying to push squeaky door, tossing a grenade into A, only to have squeaky door close on me, the grenade bounce back right to my feet, and getting blown up. Which was impressive because grenades svcked in Counter-Strike.

I was working as a Forest Guardian in Dark Souls, where you invade players that trespass in the Darkroot Garden. In this incident, I had been summoned to fight a trio of gankers. Naturally, this fight was not rigged in my favor, but nevertheless, I was determined to at least try and do my best. First thing I did was backflip away to avoid whatever starting attack the jerks would try.

Unbeknownst to me, I spawned at the edge of a cliff...

Well I wasn't too accustomed to Apple/Mac games way back when, and in the original Prince of Persia friends laughed to no end at how often I kept falling onto spikes and got my ass kicked by the very first enemy encountered. That was pure hilarity to them for hours. Of course, they weren't laughing so much later on in Doom when I'd farmkill them. :D

Way back when I started playing EQ(around the Kunark era) I made a bard. I was exploring eastern Freeport when I fell in the water and sank like a rock. I had no clue how to go up, and drowned. I never found the corpse so I just made another character, lol.

Then I drowned on THAT character in the ridiculously shallow water of the river in front of the monk's guild in Freeport. At least I found my corpse that time!

Getting killed by Radroaches in beginning of Fallout 3. Twice. (okay the first one was a bug where I couldn't draw weapons)

Any time in Red Orchestra where I try to lob a grenade in a window, I fail and blow up myself and a comrade or two.

Throwing a grenade in RO, then for some reason running into the same place where I threw it and thus dying in my own nade.

Finding out I could shoot my horse while riding it in rdr had its moments. I remember flying off a cliff and flying into water and drowning.

Jumped off of a tower after upgrading my parachute capacity in Assassins Creed Revelations. Went opposite of the leap of faith jump area and proceeded to press x to no avail as I quickly fell 200+ft to my desynchronized death...apparently upgrading your capacity depletes all your chutes in one go...little did I know...

Warcraft 3 Frozen Throne Last Mission (on highest difficulty of course)
The problem with this mission is not that Arthas (that undead cripple) starts at level 1 or that you don't have any flying units (because Illidan uses cheap tactics and got his main base safe behind deep water ,you can't cross deep water).
The problem is, if you waist to much time killing the bloodelfes or the other Nagas, Illidan will spawn after 30 secs with a constant attacking army.
Well, I didn't die, but I had to restart the mission after I waisted about 2 or 3 hours on that.
