You shot it so hard it went the whole way around the world back to where you were.

Most embarrassing? Running past my own (frag) mine in DE:HR. That was after a carefully laid plan was put into motion - I was going to lure the gangers out and into my killing corridor. I almost got spotted so I moved too quickly back around a corner and... BOOM.

ugh

Also in GTA:SA, when you have to toss molotovs through the windows of a house? Yeah. I set myself on fire three times in a row and died.

The 100s of times I've fallen off of ledges. But my favorite one being from carrying too much money in EQ and dying from a 1ft fall.

Never gotten "embarrased" over dying.

But once, I shot muself in the leg with a shotgun, and death was the result. Don't remember the game, some kind of FPS.

I died in a most unusual manner in Red Orchestra.

In Danzig 1945 me a Soviet Infantryman was approaching a wine cellar. In the staircase down was two Germans. I shot the two but one threw a grenade behind me. I dived in down the stairs and managed to kill myself due to fall damage. It must have been a sight.

Note to self:
Enchanting a boomerang spell to arrow is a bad idea, must find other means of retreaving arrows from distance.

Being knifed in CS is usually pretty bad, but being knifed when surfing has to be the worst. You don't have to be bad to get knifed while surfing, but someone has to be many, many times better than you. Especially when block is on.

On my first attempt at a very-hard hardcoe mode dead is dead play-through of New Vegas I was killed by the Geckos during the tutorial. It was mainly due to my lack of skill using the laptop touch pad with the New Vegas, but still, it was the tutorial.
